Liposuction is the surgical removal of localized or regional excess fatty tissue, it’s a highly individualized procedure that requires careful consideration of your specific concerns and the areas in which you desire improvement.  An evaluation of skin elasticity and tone, as well as fat density and distribution will determine appropriate techniques for your specific case.

All liposuction procedures require incisions in or near the area to be auctioned for insertion of an appropriately sized cannula.  Once the cánula tip has loosened excess fat, the fat is removed using a surgical vacuum or powered suction device.

Manual auctioning with a syringe is more commonly used when areas of fat removal are small and very isolated or when it is planned that fat will be processed and reinfected in other areas of the face or body.

This procedure is called fat grafting and may be used to fill-in wrinkles or hollow areas of the face or depressions appearing anywhere on the face or body. Liposuction is performed through small, inconspicuous incisions that are hidden within the natural creases and contours of the body.  A thin hollow tube, or cannula, is inserted through these incisions using a controlled back and forth motion to suction out the dislodged fat.

Sometimes liposuction is performed with other plastic surgery procedures such as tummy tuck, male or female breast reduction, etc.

What are the different types of liposuction?

Ultrasound assisted lipoplasty or UAL is also known as VASER or 4D High Definition liposuction. This technology uses ultrasonic energy,or high energy sound waves, to emulsify the fat tissue.  The soundwave vibration gently disengages and loosens the fat cells.  Once the emulsification has taken place, a small cannula is inserted to remove the fat cells and fluid used in the process.  This method is primarily used for fat removal in hard-to-treat areas such as the chin, neck, cheeks, knees, calves, and ankles. It can also treat areas that have denser fat deposits with the ability to remove larger volumes of fat at one time. The results are smoother than with the traditional liposuction and that is why it is the method used for the high definition procedures.

The tumescent or superwet liposuction technique requires an infusion of saline solution, with added adrenaline and possibly lidocaine prior to the removal of excess fat. This technique helps to reduce bleeding and therefore bruising. It can also help reduce any discomfort or pain.

What liposuction can and cannot do

Liposuction is not a treatment for obesity or a substitute for proper diet and exercise. It does not remove excess skin. It is also not an effective treatment for cellulite, the dimpled skin that topically appears on the thighs, hips or buttocks.

Liposuction can, however, permanently reshape body contours where excess fat deposits create areas that are disproportionally large in an otherwise balanced figure. Your skin elasticity plays a significant role in the overall degree of change that can be acheived with liposuction.

Am I a good candidate?

• You are in good health
• You do not smoke
• Your pre-op lab work is normal
• You are within 30% of your desired weight
• You have firm, elastic skin with good muscle tone.
• You are willing to follow post-operative instructions

Prepping for your surgery

Consultation
During the consultation, your medical history will be taken in order for Dr. Sigler to evaluate your general health.  A careful examination will also be conducted.  Together with Dr. Sigler, you will discuss the procedure and communicate your desires, your concerns and the results you hope to achieve. You may bring photographs to show Dr. Sigler examples of what you are looking for. Dr. Sigler will go over details of the procedure itself and the possible risks and complications as it pertains to any medical or structural issues she sees in your case.  She will go over the type of anesthesia to be used. Once you have all your questions answered, you will both agree on a surgery plan and a proposed date. Photos are taken before and after the surgery in order to determine the amount of improvement.

If You Smoke
If you smoke, this habit should be stopped at least two months prior to surgery. This includes vaping and marijuana smoking.

Two Weeks Prior to Surgery
Review and download this list of medications and supplements to avoid. Stop taking any medication on this list two to three weeks before the surgery.  Avoid any intake of aspirin, ibuprofen, Naproxen (excedrin, Motrin, Aleve, Advil), vitamins that include vitamin E, and energy drinks as well. Arrange for family or friends to be available for assistance when you return home to recuperate. Clear your schedule. Make a list and organize your environment to be safe and conducive to bedrest. Stock your kitchen and plan ahead for a period of time spent watching TV, reading or simply relaxing.

Labwork
Pre-op labwork should be done two weeks before the surgery with your own doctor, and then sent to Dr. Sigler. If you prefer Dr. Sigler to do the labwork, you will need to make arrangements prior to your surgery date. Required labwork includes:   CBC (Complete Blood Count), CMP (Chemical Metabolic Panel), PT (Prothrombin Time) and INR, PTT (Partial Thromboplastin Time) and any other labwork specific to your particular medical condition(s), such as a Thyroid Hormonal Panel.If you are over 50, it will be necessary to have a cardiovascular assessment including an EKG and possibly a chest x-ray. Dr. Sigler will advise.

Approval for Surgery
If you are currently undergoing treatment for a medical condition or disease, a letter from your treating specialist must be sent to Dr. Sigler at least two weeks prior to the surgery stating you are cleared to go ahead with the procedure, and include any pre-op and post-op recommendations in order to avoid possible complications during or after the surgical procedure.

Night before Surgery
You are required to fast for at least 8 hours prior to surgery. Eat a lighter dinner and do not eat or drink past midnight.Day of SurgeryDo not eat or drink anything except a little water. Allow sufficient time to get to the surgery center. Using Dr. Sigler’s car service is usually the most efficient way to ensure a timely arrival. Intake is typically scheduled two hours prior to the actual surgery. You will meet with Dr. Sigler for your pre-op consultation, pre-op photos and markings on body or face, and for a final discussion about your procedure. Paperwork is completed and final prepping by your surgical team is carefully performed.

Post Surgery Care

After surgery, a compression garment or elastic bandages may cover the treatment areas. These assist to control swelling and compress the skin to your new body contours.  In addition, small temporary drains may be placed in incisions beneath the skin to remove excess fluid. Before you are released following surgery, you and an accompanying family member, friend or caregiver will be given specific instructions that may include:

-How to care for the surgical site
-Medications to take orally to aid healing and reduce the potential for infection
-Specific concerns to look for at the surgical site or in your general health
-When to follow up with your plastic surgeon.

Follow all instructions carefully — this is important to the success of your outcome.  In general, a return to light, normal activity is possible as soon as you feel ready and is encouraged to prevent blood clots and other complications.  Initial wound healing may take from 5 to 10 days, meanwhile there could be some oozing of light red colored fluid from the wounds.  Cover them with enough gauze’s or pads to avoid staining of your clothes or bedding. Stitches will be removed between one and two weeks.  Attend follow up visits as scheduled.

The use of compression garments is a must and should be worn during day and night, continuously, until one month after the surgery.  You will be encouraged to control your weight, as significant gain weight can reverse your results.

Results and outlook
It may take several months for swelling to fully dissipate.  As it does, your new contours and enhanced self-image should continue to develop.The fulfillment you feel from the initial results of liposuction should continue so long as you maintain your weight and practice a healthy lifestyle that includes eating well and regular physical activity.
